How truly "safe" is a file or prog if the .exe is partially flagged as positive for malware by Virus Total?

Theoretically speaking of course, say you have a file or little proggy that Virus Total flags 10/60 (obscure AV positives). Why does one antivirus consider a file or program malicious and another doesn't?
BTW, I am not a programmer

10/60 is pretty high. Are they hueristic matches? Are any of the 10 from a well known virus scanner?
